Purpose: Xanthelasma palpebrarum (XP) is the most common cutaneous xanthoma characterized by yellowish plaques occurring mostly near the inner canthus of the eyelid, more often on the upper, rather than the lower lid. Patients and methods: This quasi-experimental study was conducted to assess the efficacy and tolerability of trichloroacetic acid concentration % 50 the treatment of patients with xanthelasma. The study was carried out on 30 patients attending Mansoura ophthalmic center and suffering from eye lid xanthelasma. The patients were treated with Tricholroacetic acid 50%. Plasma lipid and cholesterol levels were estimated in a lab, periorbital clinical picture to capture the periorbital anatomy and current eyelid. Results: Our study results have revealed that the age ranged between 34–56years. The size of the lesion ranged between 3.3–5.7cm2 with mean value 4.61±0.712cm2. Sites of lesion show that 10(33.3%)were left, 4(13.3%) were right and 16(53.3%) were bilateral. The Plasma lipid levels of the studied group ranged between 100–144 with mean value 121.17±14.059;22(73.3%) had normal Plasma lipid levels and 8 (26.7%) had high Plasma lipid levels. The Cholesterol level of the studied group ranged between 174–351with mean value 217.80±51.655; 21(70.0%) had normal Cholesterol level and 9(30.0%) had high Cholesterol level. Regarding the patient’s satisfaction of the studied group, 26(86.7%) were Satisfied and 4(13.3%) were Unsatisfied. Conclusion: TCA 50% administration is an extremely economical method that may be utilised in ordinary clinical practice, even in a basic care health centre, to treat XP. It also doesn't require any specific equipment.
